Payment Methods

Payment is due at the time of visit by credit card, Venmo, check or cash. If the client does not show up for their scheduled appointment, their card or package will be charged in full.

Please note that late arrival for the appointment may result in a shorter session time.

Packages

Discounted packages are available for purchase. Packages may not be combined with other offers or discounts. All packages are non-refundable and are meant to be used within the designated time frame.

24-hour Cancellation Policy

Tenth House Health requires 24-hour notice if the client chooses to reschedule or cancel an appointment for any reason other than illness (please see the covid policy). If the client cancels with less than 24-hour notice or does not show up to the appointment the client agrees to pay the full cost of the session and allows Tenth House Health to charge the credit card on file.

Covid exception:  the client suddenly experiences any symptoms that may be indicative of COVID-19 or have been exposed to someone infected with COVID-19 less than 24 hours before their scheduled appointment, they will be offered a make-up (valid for 2 months) from the time their card is charged.

Covid Policy

The client acknowledges that Tenth House Health has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19, and it is a condition of entry to comply with Tenth House Health’s COVID-19 Health and Safety Plan.

The client further acknowledge that Tenth House Health cannot guarantee that they will not become infected with the Coronavirus/COVID-19. 

The client acknowledges that they must comply with all set procedures to reduce the spread while attending the appointment.

The client agrees to not enter the studio if: 

  • They are experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or loss of taste or smell.

  • They have traveled to a highly impacted area within the United States of America in the last 14 days.

  • They have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19 and have not yet quarantined or received a negative test.

  • They have been diagnosed with Coronavirus/COVID-19 and not yet cleared as non contagious by state or local public health authorities.

Privacy Policy

The client’s data will not be sold or used for any other purpose than communications from Tenth House Health.

The entirety of the Tenth House Health website is copyrighted as a collective work under U.S. copyright laws. The content of Tenth House Health is intended solely for personal, noncommercial use by the users of our site. Tenth House Health reserves complete title and full intellectual property rights in any content downloaded from this website. Users may not copy, download, reproduce, modify, publish, distribute, transmit, transfer or create derivative works from the content without first obtaining written permission from Tenth House Health.

Client Consent/Waiver of Liability

Please fill out the Client Consent/Waiver of Liability form (which also includes the Cancellation Policy and Payment Policy) if the client has not already done so. The following text is for your reference":

“I understand that the sessions provided at Tenth House Health involve a variety of manual and techniques that manipulate the muscles, soft tissues and/or cranial sacral system and movement sequences designed for therapeutic support. 

I further understand and acknowledge that in no way are these services meant to be construed by me as the diagnosis or treatment of disease, but rather as an aid in the healing process. I understand that prior to my first session, I will receive a verbal explanation and description of the session I will receive. I understand that I may refuse any and all techniques at any time. I understand the practitioner may recommend remedial exercises and home care. I assume full responsibility for my choice to use or apply any portion of the information or instruction I receive from my practitioner.

I understand that Craniosacral therapy, manual therapy, and movement sequences are not substitutes for medical intervention. 

I understand that my practitioner will discuss potential side effects from treatment with me and furthermore will not hold the practitioner responsible for any and all normal side effects.

Because our practitioners adapt their work to specific medical conditions, I affirm that I have stated all my known history, current symptoms and diagnoses and answered all questions honestly. I agree to keep the therapist updated as to any changes in my medical profile and understand that there shall be no liability on the therapist's part should I fail to do so.

By submitting this form I affirm the accuracy of the information I have provided and understand and agree to the policies above. 

I declare that I have given the most accurate information to my knowledge, and I understand that the information given will be used to create a personal treatment plan and create a client file. I therefore give my consent to the therapist to perform this assessment, and treatment and to collect and use my personal information.

Payment Policies

I understand and am aware of the posted fees and cancellation policy and that payment is due at the time of session. I understand that late arrival for my appointment will result in a shorter session time. 

I am aware that Tenth House Health is not responsible for any lost, stolen or damaged articles.

I am aware that all information provided is private and confidential and will not be released without my written consent. 

Cancellation Policy

Tenth House Health has a 24 hour cancellation policy. If you need to change or cancel your appointment, you must notify us at least 24 hours in advance. Missed appointments and those cancelled without the required 24 hours notice will be subject to the full cost of the appointment. 

Current exception:  if you suddenly experience any symptoms that may be indicative of COVID-19 or have been exposed to someone infected with COVID-19 less than 24 hours before your scheduled appointment, you will be offered a make-up appointment (valid for 2 months) for the charge for the cancellation.
